The following quote is found in the Vachaspatyam:

“मायोपाधिरयं जीवः: अविद्योपाधिरीश्वर” इत्युक्तेः...

There is, however, a well known quote:  कार्योपाधिरयं जीवः कारणोपाधिरीश्वरः।
in the Shukarahasyopanishad. But the above cited one is not seen to be in
any text.

The source of the quote is not known.  Any help is appreciated.
